Non-Governmental Bar Date: On May 18, 2017, the Bankruptcy Court established July 31, 2017 as the date by which Proofs of Claim (including 503(b)(9)) must be filed. Claims filed electronically may be filed up to 11:59:59 p.m. Eastern. All other claims must be filed by delivery to the Clerk’s Office on business days between 8:00 a.m. and 4:00 p.m. Eastern. All Proofs of Claim shall substantially conform to Official Form 10.

You must specify the Debtor against which the claim is being asserted and must file the proof of claim in the specific bankruptcy case pertaining to that Debtor. If one or more Debtor is alleged to be liable, then a proof of claim must be filed in the bankruptcy case for each of the allegedly liable Debtors.
